- Centrelink and NDIS related paper work for new and inactive patients. (E.g. Centrelink medical certificate, Disability Support Pension application, Carer Payment/Allowance medical reports and NDIS Access Request Forms)
- Instant and telehealth Mental Health Care Plan. (Please note that patients need to consult with the doctor who will determine the eligibility and will arrange for subsequent appointment)
